How it works

Work through a structured series of questions covering the key elements of your Lockout Tagout system.  For each question, consider your current arrangements and the evidence available within your workplace.

Your responses are assessed across the nine critical elements.  Once complete, you’ll receive your results showing where your programme is performing well and where improvement should be prioritised.

What do the Audit Standards Cover?

The audit standards cover the following critical elements:
1. Programme Governance & Management

Ensure the LOTO program is formally structured, endorsed, and communicated, with clear roles and responsibilities for safe energy control.

2. Risk Assessment & Energy Identification

Identify and assess all hazardous energy sources to guide safe isolation and lockout procedures.

3. Written Procedures & Documentation

Maintain clear, standardised procedures to guide safe lockout/tagout operations and ensure consistency across all equipment and tasks.

4. Authorisation & Trainining

Ensure workers are trained, competent, and authorised to perform their specific LOTO responsibilities safely and effectively.

5. Lockout/Tagout Hardware & Tools

Ensure that adequate, reliable, and standardised hardware is available and properly used to safely isolate energy sources.

6. Implementation & Operational Practices

Verify that LOTO procedures are consistently applied during daily operations to ensure equipment is isolated and work can be safely performed.

7. Monitoring, Auditing & Continuous Improvement

Regular monitoring, auditing, and review of LOTO practices ensure the program remains effective, compliant and continuously improves over time.

8. Documentation & Record Keeping

Accurate, complete, and accessible records of all LOTO activities are critical for accountability, auditing, and demonstrating compliance with best practices and standards.

9. Programme Culture & Awareness

A world-class LOTO system relies on more than procedures and hardware — it requires strong leadership commitment, workforce engagement, and ongoing awareness to ensure consistent safe behaviours.

Maturity Score

How Your Score Is Determined

Your assessment results are mapped across nine critical performance areas to evaluate the maturity, consistency and effectiveness of your Lockout Tagout Programme and identify opportunities for improvement.

Important

Your maturity score and radar graph do not determine legal or regulatory compliance.

They are intended as developmental tools to help identify strengths, highlight gaps and guide continuous improvement against recognised good practice — not as compliance certification.
